Support residents with rehabilitation goals, independence, and community engagement.
Work collaboratively with a multidisciplinary team to implement tailored care plans.
Build positive, trusting relationships with residents, their families, and colleagues.
Monitor and report changes in residents’ conditions, ensuring their safety and well-being at all times.
