Summary
Home Visits: Conduct clinical assessments and treatments in patients homes, particularly for housebound and vulnerable individuals. Minor Illness Clinics: Diagnose and manage minor illnesses and injuries independently or with GP support. Triage and Assessment: Support telephone and face-to-face triage, assessing patients and directing them to appropriate care pathways. Chronic Disease Management: Assist in monitoring and managing long-term conditions such as asthma, COPD, diabetes, and hypertension. Emergency Response: Provide urgent care and support in emergency situations within the practice setting. Medication Reviews: Conduct reviews and support prescribing within scope of practice. Health Promotion: Educate patients on lifestyle, prevention, and self-care strategies. Documentation: Maintain accurate and timely clinical records using EMIS or other clinical systems. Collaboration: Work closely with GPs, nurses, pharmacists, and other allied health professionals to ensure coordinated care. Safeguarding: Recognise and act on safeguarding concerns in line with local and national policies.
