Summary
Key Responsibilities and Duties: The PCN ACP areas of responsibility include: Providing same day urgent response to housebound patients Collaborating with a clinical team to ensure care home residents receive co-ordinated medical care. This includes proactive care planning and consistent support from a multidisciplinary team Participation in multidisciplinary team meetings Recognition of opportunities for anticipatory care of suitable patients via treatment plans Working with registered practice to ensure seamless care. Leading and coordinating across organisations. Delivering high quality, methodical and well-informed patient diagnoses and care plans. Developing expertise within the community for improving the lives and care of the elderly. Promoting the use of supportive, non-statutory services to support self-care and social prescribing agendas. Maximise best care in the patients care home in order to reduce the need for hospital admissions. Relationship Management: Local clinicians, including Geriatricians, ACPs, Enhanced Clinical Practitioners, GPs, members of the local integrated community service, out of hours and urgent care providers, care coordinators and care home workforce. Local providers of health and social care including the Local authority, GP practices, and local integrated community service Local hospitals, community mental health and Community health services provider.
