Summary

Job Summary: The post-holder will manage a personal caseload of patients with a long term condition and other routine patients and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients. Clinical Responsibilities of the Post Holder: -In accordance with the Practice timetable, as agreed, the post-holder will make him/her-self available to undertake a variety of duties including surgery, online and telephone consultations and queries, visiting patients at home, signing repeat prescriptions and dealing with queries, insurance forms, paperwork and correspondence in a timely fashion -To be part of an equitable rota for on call duties including covering for absent colleagues. -To make professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation -To be involved in the training of any clinicians or students within the practice provided that the Partnership has made provision for the increase in workload. -To assess the health care needs of patients with undifferentiated and undiagnosed problems -To be responsible for the overall management of their personal caseload and endeavour to ensure that their case load of patients have had appropriate Care Plans and met the local targets set out by the Practice -To work with the wider community health and social care teams, especially Chiswick Primary Care Network -To screen patients for disease risk factors and early signs of illness -To provide counselling and health education -To record clear and contemporaneous consultation notes to agreed standards -To act on patient status alerts, reminders or recalls whether or not they are the usual GP for that patient -To continuously be responsible for maintaining the computer records of patients that they see with a particular emphasis on their caseload. -To collect data and participate in any surgery audits -To compile and issue computer-generated acute and repeat prescriptions -To prescribe in accordance with the Practice prescribing formulary (or generically) whenever this is clinically appropriate -To be responsible for QOF and other targets of those patients that consult with them -To be responsible for adding patients to the Extended Contract templates as necessary and for entering and maintaining the appropriate data on those templates. -To maintain concise Major and Minor views on SystmOne on patients for whom they are the usual GP -Other Responsibilities within the Organisation -Awareness of and compliance with all relevant Practice policies/guidelines, e.g. prescribing, confidentiality, data protection, health and safety, including CQC regulations. -A commitment to life-long learning and audit to ensure evidence-based best practice -Contributing to evaluation/audit and clinical standard setting within the organisation -Contributing to the maintenance of computer-based patient records -Contributing to the summarising of patient records and Read-Coding patient data -Attending training and events organised by the Practice or other agencies, where appropriate. -Contribute to the education of medical students, training doctors, pharmacists or AHPs when they are in the Practice, including where appropriate allowing them to sit in your clinic -Participation in an annual individual performance review, including taking responsibility for maintaining a record of own personal and/or professional development -Taking responsibility for own development, learning and performance and demonstrating skills and activities to others who are undertaking similar work. Contribution to the Implementation of Services: The post-holder will: -Apply Practice policies, standards and guidance -Discuss with other members of the team how the policies, standards and guidelines will affect own work -Participate in audit and clinical review. -Work with Partners and the Management team to achieve standards of quality, performance standards, budgets and targets without compromising levels of patient health care -Contribute towards the development of the Practice as a member of Chiswick Primary Network and the developing Hounslow Integrated Care Partnership -Contribute to maintaining or improving the practice standing on measures such as the GP outcome framework, Clinical Dashboards etc
