Summary

About This Exciting Opportunity We're seeking a dedicated Care Coordinator to join our Personalised Care Team, specialising in safeguarding support for children, young people, families, and vulnerable adults. This role is integral to our mission of providing coordinated, patient-centred care whilst ensuring the safety and wellbeing of our most vulnerable patients. What Makes This Role Special As our Care Coordinator (Safeguarding), you'll be at the heart of ensuring our most vulnerable patients receive timely, coordinated care. You'll work across our seven GP practices, collaborating with an established multidisciplinary team to implement evidence-based safeguarding plans and improve patient journeys through primary and secondary care. Your Key Responsibilities Care Planning and Coordination You'll be actively involved in introducing, developing, and reviewing safeguarding care plans, ensuring they're discussed with relevant professionals and outcomes are appropriately recorded. You'll ensure these plans are communicated effectively to GPs and uploaded to relevant online care records using appropriate coding systems. Administrative Excellence A crucial aspect of your role involves completing initial patient note reviews and preparing the first draft of child protection reports for safeguarding meetings. You'll work closely with safeguarding lead GPs who will provide final oversight and sign-off on these important documents. Communication and Relationship Building You'll establish and maintain effective communication networks with clinicians, team members, families, and external agencies. Working closely with our Young People and Families Coordinator and Social Prescribing Link Workers, you'll ensure seamless coordination of support services. Training and Development Support You'll play a key role in developing and coordinating safeguarding training needs across our practices, maintaining accurate training records, and helping disseminate important updates from the ICB team regarding policy changes and training opportunities. Quality Improvement You'll identify delays in care delivery, map processes to identify bottlenecks, and work with all stakeholders to establish good working relationships that improve patient journey coordination. Working Environment and Support You'll be supported by a named clinical point of contact and work within our established governance frameworks. Regular attendance at quarterly safeguarding leads meetings will keep you connected with the wider safeguarding community and ensure you're up-to-date with best practices. Training in MS Office and web-based services will be provided as needed. Location & Travel Requirements While based at Easingwold Health Centre, you'll be required to travel flexibly across all seven of our GP practices (Pickering, Kirkbymoorside, Helmsley, Terrington, Stillington, Tollerton, and Millfield) to deliver services and support patients in their local communities. This role therefore requires a current full driving licence and sole use of your own vehicle.
