Description

Responsibilities
Conduct comprehensive assessments of clients in accordance with agency standards.
Develop and implement individualized service plans to meet clients' needs.
Provide counseling and support to individuals and families dealing with social and emotional challenges.
Collaborate with healthcare professionals, community organizations, and other stakeholders to facilitate service delivery.
Advocate for clients' rights and access to necessary resources and services.
Monitor and evaluate clients' progress and adjust service plans as needed.
Document all client interactions and maintain accurate case records in compliance with regulatory requirements.
Requirements
Master's degree in Social Work (MSW) from an accredited program.
Current Licensed Clinical Social Worker (LCSW) or equivalent licensure in the state.
Experience working with diverse populations in a social work or healthcare setting.
Strong knowledge of social work principles, practices, and applicable laws.
Excellent interpersonal and communication skills, both written and verbal.
Ability to work collaboratively in a multidisciplinary team environment.
Commitment to ongoing professional development and staying updated on best practices.
Hours Per Week: 37.00
Start Time: 09:00
End Time: 17:30
Pay Per Day: £225.00
