Summary
JOB DESCRIPTION
This is a pivotal leadership role within the Hospital Social Work Service, based at University Hospital Ayr and responsible for social work intervention across all hospital sites. The Team Leader will oversee a dedicated team of social workers who support safe and timely hospital discharge, working closely with health colleagues and community services to ensure person-centred planning and continuity of care. This post offers the opportunity to lead and shape hospital-based social work practice, promote effective multi-disciplinary working, and contribute to improved outcomes for individuals during periods of acute health need.

REQUIREMENTS
