The Band 6 Mental Health Screenerplays a vital role in the Community Mental Health Team (CMHT), conducting initial assessments, evaluating mental health needs, assessing risk, and supporting care planning. The post holder also contributes to team development, clinical supervision, and workforce planning, ensuring timely access to care and maintaining high clinical standards.

1. Conduct initial mental health assessments using appropriate tools and frameworks.
2. Formulate clinical impressions and care needs based on assessment findings.
3. Assess and manage clinical risk, including safeguarding concerns.
4. Develop and review Dialog plus in collaboration with service users and carers.
5. Deliver therapeutic interventions appropriate to the service user’s needs.
6. Liaise with GPs, social care, and other agencies to coordinate care.
7. Monitor and support service users’ physical health needs.
8. Maintain accurate and timely clinical documentation.
9. Apply relevant legislation including the Mental Health Act and Mental Capacity Act.
10. Promote recovery-focused and person-centred care.

In order to meet the needs of the Trust you may be required from time to time to work at different locations to your normal place of work. This may mean that you are required to work at any location that fall under Barnet, Camden, Enfield, Haringey or Islington. The Trust reserves the right to require staff to work at such other places or locations as it considers reasonable and necessary on a temporary or permanent basis.
Join Our Dynamic Community Mental Health Team as a Band 6 Screener
Are you a passionate and experienced mental health professional looking to make a real difference in the community? We are excited to offer an opportunity for a Band 6 Screener to join our dynamic and supportive Community Mental Health Team (CMHT).
As a Band 6 Screener, you will play a vital role in the initial assessment and triage of referrals into the CMHT. You will be responsible for identifying the most appropriate care pathways for individuals experiencing complex mental health needs, ensuring timely and effective support.
This is a frontline clinical rolethat requires strong decision-making skills, compassion, and the ability to work collaboratively within a multidisciplinary team.
