Summary

Clinical Pharmacist – Primary Care Network (Nuneaton)

We are seeking a motivated and experienced Clinical Pharmacist to join our dynamic primary care team in Nuneaton. This is a practice-based role within a collaborative network, offering the opportunity to make a real impact on patient care.

Key Responsibilities:

Conduct clinical medicines reviews, focusing on high-risk patients such as those with polypharmacy, frailty, renal or hepatic impairment, and complex medication regimens.

Reconcile patient medications following hospital discharge and liaise with community pharmacy colleagues to ensure continuity of care.

Provide expert advice to prescribers, patients, and healthcare teams on medication use, safety, and alternatives.

Lead initiatives to improve prescribing practices, implement clinical guidelines, and support quality improvement through audits.

Manage repeat prescriptions, electronic repeat dispensing, and re-authorisation within scope of practice.

  • Contribute to clinics for long-term condition reviews, multi-morbidity assessments, and patient education.


  • Act as a medicines information resource for the practice team and patients.


    Requirements:

  • Registered Pharmacist with GPhC registration and membership of the Royal Pharmaceutical Society.

  • Minimum 2 years post-graduate experience in pharmacy, including at least 1 year in primary care or general practice.

  • Experience in clinical assessment, medicines optimisation, and working in multidisciplinary teams.

  • Strong communication, influencing, and relationship-building skills.

  • Desirable: independent prescribing qualification, postgraduate pharmacy qualification, EMIS experience, and supervisory experience.
