Summary
Leadership Responsibilities Provide professional advice and support to practice nursing teams. Offer guidance to maintain and improve standards of care. Participate in chronic disease management and provide practical assistance to achieve high standards of care. Provide clinical input alongside other practice nurses, particularly new staff, to support skill development. Delegate tasks appropriately based on competence and knowledge. Maintain evidence-based nursing practice. Serve as a resource to patients, families, carers, and external healthcare professionals. Research, Audit & Quality Support practice audit strategies, action plans, and evaluation. Contribute to equitable, high-quality services for patients in the locality. Assist in integration across primary, secondary, and tertiary care services. Keep up to date with national research initiatives (e.g., NICE) and disseminate relevant information. Ensure all care is evidence-based using up-to-date validated literature. Education & Training Provide advice and support to primary healthcare professionals. Participate in training, facilitation, and clinical supervision of other practice nurses. Mentor and guide junior staff. Contribute to awareness, detection, and prevention of chronic diseases. Maintain expertise in chronic disease management (Diabetes, COPD, Asthma) and develop new skills. Advise on health promotion, prevention, and protection. Pursue continuous professional development, including in-house training and monthly PLT. Participate in multidisciplinary education and training. Direct, supervise, and mentor junior nursing staff. Monitor professional standards and take appropriate action if not met. Maintain professional development and NMC registration. Participate in annual appraisal. Main Responsibilities Meet patients nursing care needs, including occasional home visits. Initiate and maintain health screening and promotion clinics. Manage chronic disease care using personal recall systems and practice computer systems. Clinics offered include: o Asthma / COPD o Diabetes o Hypertension o Cardiovascular Disease o Cervical Smears o Family Planning o INR Monitoring Maintain liaison with Primary Care Team and external agencies for continuity of care. Lead infection control Routine Treatment / Clinical Duties: Wound management (dressings, suture/clip removal) Blood tests, ECGs, venepuncture Health promotion and lifestyle advice (nutrition, BMI, smoking, exercise) Infection control and emergency management (anaphylaxis, resuscitation) Womens health awareness (cervical cytology, mammography) Accurate record-keeping and data management Patient liaison and complaint escalation Additional Skills: Prioritization and balance of administrative and clinical tasks Contribution to practice policies and growth Excellent written and verbal communication Effective use of resources and decision-making Working knowledge of clinical software and basic IT skills Professional Registration Maintain annual registration with the NMC. Adhere to the NMC Code of Professional Conduct and professional accountability. Governance Take part in the maintenance of quality governance systems and processes across the Practice and its activities. Utilise the audit cycle as a means of evaluating the quality of the work of self and the team, implementing improvements where required. Work with other teams on improving the quality of service and patient care, in response to local and national policies and initiatives as appropriate Support and participate in shared learning across the practice and wider organisation Manage, review and identify learning from complaints, incidents and near-miss events relating to the practice, team and self. Awareness of statutory safeguarding, notification processes and local guidance for children/vulnerable patients, applying relevant policies and legislation to protect them Ensure compliance with policies, procedures and guidelines for self and others, by acting or alerting senior management team if the practice appears to contravene policy, or if there are concerns over any aspect of patient care. Confidentiality Maintain confidentiality of information, acting within the terms of the Data Protection Act and Caldicott guidance on patient confidentiality at all times. Maintain an awareness of the Freedom of Information Act. Information relating to patients, carers, colleagues, other healthcare workers or the business of the practice may only be divulged to authorised persons in accordance with the Practice policies and procedures relating to confidentiality and the protection of personal and sensitive data Health & Safety The post-holder will manage their own and others health & safety and infection control as defined in the Practices Health & Safety Policy, the Practice Health & Safety Manual, and the Practices Infection Control Policy and published procedures. Comply with Practice health and safety policies by following agreed safe working procedures Actively reporting of health and safety hazards and infection hazards immediately when recognised Keeping work and general areas clean and tidy, and using appropriate infection control procedures to keep work areas hygienic and safe from contamination. Undertaking periodic infection control training (minimum annually) Awareness and compliance with national standards of infection control, hygiene, regulatory / contractual / professional requirements, and good practice guidelines. Correct personal use of Personal Protective Equipment (PPE) and ensuring correct use of PPE by others, advising on appropriate circumstances for use by clinicians, staff and patients. Reporting incidents using the organisations Incident Reporting System Using personal security systems within the workplace according to Practice guidelines Making effective use of training to update knowledge and skills Equality And Diversity The post-holder will support, promote and maintain the Practices Equality & Diversity Policy.
