Summary


Due to the continued success of UHL’s Frailty Same Day Emergency Care (SDEC) pilot, we are excited to offer opportunities for motivated and experienced Band 6 Physiotherapists to join our innovative Rapid Assessment and Discharge Service (RADS) team.

Frailty SDEC is a forward-thinking model of care designed to deliver timely, multidisciplinary assessments and interventions, outside of the Emergency Department, to older adults presenting with frailty, enabling same-day discharges and reducing unnecessary admissions.

As part of the RADS team you’ll play a key role in delivering high-quality, person-centred assessments across the Emergency Department, Acute Assessment Units as well as Frailty SDEC. You will work collaboratively with a large multidisciplinary team (MDT) to provide comprehensive care supporting safe, timely discharges with robust therapy input and community liaison. Furthermore, as a team we are passionate about promoting independence and preventing deconditioning for all patients across the Emergency Floor through early therapy intervention, effective MDT working and teaching.

We are a supportive and friendly therapy team with flexible working opportunities as the service operates 12 hours a day, 7 days a week. You will receive regular supervision, yearly appraisal and regular in-service training. There will be opportunities for continual professional development across the four pillars of practice. You will be encouraged and facilitated to be involved in service development and quality improvement.
