Summary
Conduct mental health assessments and provide brief interventions. Support patients with common mental health conditions, such as anxiety and depression. Collaborate with GPs, nurses, and other professionals to develop and review care plans. Identify risk and safeguarding concerns and escalate appropriately. Liaise with community and secondary mental health services to ensure effective referral pathways.
