Summary

Main Responsibilities To deliver urgent care on the day (face to face consultations) To deliver expert advice to patients and relatives in order to promote health and protect their well-being. Receive patients with undifferentiated and undiagnosed problems. Exercise a high degree of professional autonomy and critical judgement to make differential diagnosis using decision making and problem solving skills. To provide extended skills in the management of a wide range of minor illness/injuries and ailments. Develop with patient and/or carer ongoing nursing care plan for health, emphasising prevention, utilising the wider health and social care services appropriately. Initiate and interpret tests and investigations as appropriate. Develop and utilise skills in nurse prescribing. Refer to a broad range of professionals across health and social care internally and externally as appropriate. All referrals must be discussed with a GP Act as a patient advocate. Management of Chronic Disease (within area of speciality) Ensure that their practice is current with up to date practice and following current guidelines. Monitoring and development of individual management plan; agreeing these as appropriate with the patient and other health professionals. Recognising abnormalities and acting accordingly. Identifying the impact of treatment and implementing or recommending changes as appropriate. Check compliance and the adherence to appropriate treatments using a holistic patient-centred approach. To carry out diagnostic and therapeutic procedures. Professional Role: Maintain their professional registration Promote evidence-based practice through the use of the latest research-based guidelines and the development of practice-based research. Monitor the effectiveness of their own clinical practice through the quality assurance strategies such as the use of audit and peer review Maintain own clinical/professional expertise by participating in clinical activities, personal development planning and clinical governance. Keep up to date with current evidence-based practice. Training requirements will be monitored by yearly appraisal and will be in accordance with practice requirements. Personal development will be encouraged and supported by the practice. It is the individuals responsibility to remain up to date with recent developments Participate in continuing professional development opportunities to ensure that up-to-date evidence-based knowledge and competence in all aspects of the role is maintained Work within the latest NMC Code of Professional Conduct Record accurate consultation data in patients records in accordance with the latest NMC guidance and other pertinent standards Keep up to date with pertinent health-related policy and work with the practice team to consider the impact and strategies for implementation Work collaboratively with colleagues within and external to the practice. Demonstrate leadership Pro-actively promote the role of the NP within the Practice, and externally to key stakeholders and agencies Communication To generate and keep accurate and concise clinical records in accordance with NMC guidelines. To be aware of data protection issues particularly within a GP surgery. To liaise effectively with other health professionals both inside and outside the organisation, with regard to accepting and making clinical referrals. Communicate effectively with patients and relatives, considering differences in language, literacy and culture. As well as the nursing team there is a need to work closely with administration staff at the surgery. It will be necessary to attend and contribute to various practice meetings as requested. The only reason for not attending will be annual, study or sick leave.
