Summary

The following are the core responsibilities of the Care Coordinator. There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels: a. Assist with EMIS web clinics and coordinate rotas b. Coordinate recall process for long term conditions and reviews c. Deal with SAR requests and action appropriately within GDPR law d. Oversee administration and support operations of the practice. e. Line manage all administrative staff supporting staff development, providing guidance and direction, ensuring staff are up to date with mandatory training. f. Distributing labs and prescriptions to GPs g. Raising referrals via the NHS e-referral system and emails h. Results admin and chasing results by liaising with hospitals i. Dealing with medical report requests j. Answer incoming phone calls, transferring calls or dealing with the callers requests appropriately k. Welcome patients and visitors to the organisation, directing requests appropriately l. Always maintain a clean, tidy, effective working area m. Support all clinical staff with general administrative tasks as requested In addition to the primary responsibilities, the Care Coordinator may be requested to: a. Partake in audit as directed by the audit lead b. Support reception staff, providing cover during staff absences c. Book appointments d. Complete opening and closing procedures in accordance with the duty rota e. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels
