About this role…

We are looking for a Senior Practitioner to join the Alliance Rehabilitation Team & Primary Care Alliance Network team.

The Lambeth Living Well Network Alliance supports people in Lambeth experiencing mental illness. Thames Reach works in partnership with South London and Maudsley NHS Foundation Trust, Lambeth NHS Clinical Commissioning Group, Lambeth Council and Certitude to deliver mental health services as part of the innovative transformation of mental health services in the borough.

We will not accept applications where Artificial Intelligence (AI) has been used to assist with completing the competency questions.

You will:

.Coordinate, attend and chair all online PCAN meetings between to GPs and secondary mental health teams. Keep details notes on all discussions and circulate to relevant colleagues.
.Record and safely store data from all of the PCAN meetings, completing regular report for Lambeth Alliance management and governance colleagues.
.Present about the PCAN meetings to wider healthcare and mental health services as and when required.
.Manage a team of Lead Workers within the Alliance Rehabilitation Team delivering social, housing and financial support to adults in mental health supported accommodation placements in Lambeth.
.Work closely with colleagues from Lambeth Council Social Services, SLAM and Certitude to oversee holistic support for clients within the ART team.
.Oversee the delivery of KPIs and working closely with the wider Thames Reach Charity

You will have:

.Excellent Microsoft Office skills – Word, Excel, Teams, SharePoint.
.Experience in managing large amounts of sensitive data.
.Experience in building and maintaining strong relationships with external organisations and internal colleagues within the team.
.Understanding when to prioritise effectively, be proactive, flexible and an effective problem solver.
.Experience in developing resilience, working under pressure and to tight deadlines.
.Experience of working with vulnerable clients, ideally within the mental health sector.
