Summary
Bath and North East Somerset Council is seeking a dedicated Team Manager to join the Community Pathways Team.
In this pivotal role, you will lead a team responsible for carrying out Care Act assessments for individuals discharged from hospital back to their own homes with reablement support. You will work closely with the hospital team, which manages discharges from both community and acute hospitals, ensuring seamless collaboration across services. The role requires flexibility, with the team working across different areas as needed.
