Summary


The Quality, Governance and Assurance Coordinator (QGAC) focuses on patient safety across urgent and emergency care in the ICB area (West Yorkshire, North Yorkshire & Humber, South Yorkshire). Responsible for coordinating quality, governance, and assurance activities, the QGAC oversees incidents, patient safety events, and investigations within A&E, ensuring compliance with the Patient Safety Incident Response Framework and relevant policies.

The role involves supporting complaint resolution, service-to-service enquiries, and monitoring emerging trends to mitigate future patient harm. The QGAC works to ensure consistency in clinical governance throughout A&E and supports the Local Incident Review Group by identifying themes, managing action plans, and sharing learning across teams.

As the primary contact for patient safety Datix matters, the QGAC delivers support on investigations and governance procedures, prepares complex reports for internal and external stakeholders, and helps develop and maintain SOPs. The post-holder manages sensitive workloads, builds relationships across Trust teams, and fosters a patient-focused, safe service culture aligned with Trust values.

The successful candidates will assist the QGAMs and operational management teams with governance, quality, audit, and patient safety. Key duties include:

· Maintain accurate records and respond promptly to enquiries on complaints, incidents, safeguarding issues and Datixes;

· Work with Patient Relations to update individuals on concerns or complaints;

· Assess incident risks, and raise implications as appropriate;

· Use judgment to resolve issues, and conduct thorough investigations;

· Analyse data to identify trends and learning points, and produce clear reports;

· Review investigation responses for completeness and quality;

· Apply initiative when handling enquiries and adjust plans as required;

· Support the swift resolution of concerns and facilitate After Action Reviews per PSIRF priorities;

· Report serious issues swiftly and help maintain business continuity;

· Implement contingency plans and stay familiar with action procedures;

· Ensure compliance with policies and contribute to policy development;

· Keep Datix and other systems updated; generate reports for leadership;

· Manage administration and databases efficiently;

· Investigate complaints and incidents according to Trust policies;

· Contribute to lessons learned and improvement initiatives in governance and quality;

· Assist with leadership reports on trends and recommendations;

· Uphold confidentiality and follow GDPR, Data Protection Act, Caldicott, and legal standards;

· Comply with health and safety, training, equal opportunities, and Trust policies.

The Quality, Governance and Assurance Coordinator (QGAC) is an exciting new role within the three A&E Operations teams. The primary focus is supporting the Quality, Governance and Assurance Manager (QGAM) in collating information from patient safety events and other incidents, in order to improve patient safety. Two posts will be filled across each of the Operations areas - Humber & North Yorkshire, South Yorkshire and West Yorkshire.

The successful candidates willbe involved in the oversight and coordination of incidents and patient safety events within their relevant area - this may involve some investigative work, supporting others to do the same, and enabling the QGAM to provide assurance that incidents are dealt with in accordance with the Patient Safety Incident Response Framework (PSIRF) and other YAS policies and procedures.

The QGAC will work closely with the Patient Relations team to support the timely and satisfactory resolution and response to complaints and concerns. They will also work closely with the Area Leadership Team to coordinate any service-to-service enquiries and provide timely responses and monitor for emerging themes to aid in mitigation of future patient harm.
