Recruiting now for two Support Practitioners offering 30 hours and 18 hours per week via a supportive staff rota.

About the Outreach Service

Our community-based Outreach service supports individuals across Greater Glasgow to live independently and confidently. We offer tailored assistance with daily tasks like shopping, appointments, and household management, helping people build life skills and self-reliance.

We also encourage participation in social and recreational activities—cinema trips, café visits, and community events—fostering connection and inclusion.

Support Practitioner Responsibilities:

>Healthcare Support: Assist with personal care and medication.
>Promoting Independence: Follow personalised Support Plans.
>Record Keeping: Maintain clear, accurate documentation.
>Team Collaboration: Work closely with families and colleagues to deliver high-quality care.

The service runs 7 days a week with flexible shifts between 8:00am and 10:00pm. Support often begins in individuals’ homes across the Greater Glasgow area.
