Summary
The clinical pharmacist will be supported and mentored by our senior pharmacist and Clinical Partners. You will have regular contact with patients and will reduce the workload of GP prescribers. Your duties will include: Remote and patient facing structured medication reviews Management of long-term conditions within scope of practice Support the pharmacy technicians and prescription clerks manage acute and repeat prescription requests Telephone medicines support for patients with questions, queries and concerns about their medicines Medicine's information to practice staff and patients Management of medicines at discharge from other places of care: hospital/care homes Driving appropriate use of e- Repeat Dispensing Delivering any quality improvement activity for prescribing required by QOF or any other contractual agreements made by the PCN Contribute pharmaceutical advice for the development and implementation of new services Implement changes to medicines that result from MHRA alerts, product withdrawal and other local and national guidance. Provide education and training to primary healthcare team on therapeutics and medicines optimisation. About you: You may not have worked in general practice before, but are you a confident communicator who is able to speak to a wide range of different patients in person and over the phone? Can you manage your own workload and work as part of the team without close supervision. Are you efficient, organised and able to work under pressure? You will have to be enrolled onto the CPPE Primary Care Pathway, if you arent already you will have to go through the training course to get enrolled.
