Dementia Care Mapper
An exciting opportunity has arisen to join our Care Quality Dementia Mapping Team.
Purpose of the role:
You will be an ambassador for Dementia Care Quality by utilising a Dementia Quality Framework to provide specialist support and advice to providers, their staff, and other professionals on matters relating to the quality of Dementia Care that they deliver.
You will use a specialist tool to undertake Dementia Care Mapping to develop bespoke plans of care and support with and for people who are living with a diagnosis of Dementia that are supported by the Councils commissioned Social Care Providers.
You will work directly with people and their families, providers and a range of internal and external partners, empowering them to deliver the best possible outcomes for people living with Dementia by creating a positive Adult Social Care culture that ensures that ‘Excellence” in quality Dementia care and support is always championed.
The role works across all Adult Social Care and focuses on dementia quality assessments and Dementia Care Mapping in Residential & Nursing, Home Care, Extra Care, Supported Living and Day Opportunities settings.
You will be an integral part of a unique, specialist and critical function within ASC and lead on assessing and monitoring the Dementia Care quality delivered by commissioned providers and matters relating to people living with Dementia that are supported by commissioned providers.
You will lead on person centred values, innovative and creative practices ensuring that people living with Dementia that access services are receiving the best possible outcomes to them and ensuring that providers consistently deliver high quality Dementia care and support to meet the expectations and needs of the people they are responsible for.
You will be an integral part of a multi-disciplinary team working with Contract & Care Quality Assurance Team, Social Work Teams, Safeguarding, Health Colleagues, professionals and partners to provide a Dementia care quality and mapping service to people that are supported by commissioned providers across the city that are living with a diagnosis of Dementia.
You will have specialist skills and qualified in observing best practice and able to develop bespoke action plans to ensure providers understand and are able to identify people’s unmet needs and how to meet these in the future.
You will be experienced and confident in using professional judgement and evidence-based approaches to compile complex and comprehensive mapping and quality reports regarding people’s care and support needs and quality of Dementia care delivered and present these to professionals and providers in such a way that facilitates supportive engagement and commitment to achieving people’s individual outcomes and overall continuous service improvements.
As a member of a multi-disciplinary team, support an integrated approach to working closely with Adult Safeguarding, CHCP, Humber Teaching Trust, Alzheimer’s Society, GP’s, Social Workers, Frailty Team, YAS to maintain significant input and contribute to people’s safety, health & wellbeing and their care and support planning.
You will build positive and constructive relationships with providers to co-produce care and support plans and action plans and take on a professional role taking responsibility for the mentoring of staff and the implementation and review of innovative approaches in the delivery of care and support that people receive to achieve their individual goals in a safe and person-centred way.
Qualifications & Experience required for this role:
GCSE Grade C or above in Maths and English (or equivalent)
Level 3 Diploma in Health & Social Care or equivalent
A qualification in Dementia Care Mapping is desirable, however full training will be given to the successful candidate.
