Summary

Assess, plan, implement, and evaluate care for patients with complex wounds,including leg ulcers, and skin integrity issues. Provide specialist advice and support to patients, carers, and healthcare professionals regarding wound management and prevention strategies. UndertakeAnkle Brachial Pressure Index (ABPI) measurementsas part of holistic clinical assessment where clinically indicated. Develop and implement individualised care plans, ensuring they are evidence-based and in line with current best practices. Conduct regular wound assessments and document findings accurately in patient records. Recognise when specialist referral is required and liaise appropriately withvascular, dermatology, or other relevant services. Educate patients and their families on wound care techniques and preventative measures. Collaborate with the multidisciplinary team to ensure a holistic approach to patient care. Participate in the development and delivery of training programs for healthcare staff on tissue viability and wound management. Stay updated with the latest research and advancements in tissue viability and incorporate this knowledge into practice. Contribute to the development and review of policies and protocols related to tissue viability and wound care. Participate in clinical audits and quality improvement initiatives to enhance service delivery. Training and personal development Training requirements will be monitored by yearly appraisal and will be in accordance with PCN requirements. Personal development will be encouraged and supported by the PCN. It is the individuals' responsibility to remain up to date with recent developments. Participate in the education and training of students of all disciplines and the induction of all members of the PCN Team where appropriate. Maintain continued education by attendance at courses and study days as deemed useful or necessary for professional development. Develop and maintain a Personal Learning Plan.
