Summary


An exciting opportunity has arisen to work within the Mental Health Services for Older People Liaison Team to recruit a B6 Nurse for a 12 month fixed term period. This would be to join a dynamic, well structured multidisciplinary team who provide mental health liaison input to the North Tyneside General Hospital site & also the Royal Quays Intermediate Care Unit.

The team provides specialist input to assessment, clinical care and therapy interventions for the older person who may be experiencing comorbid physical and mental health problems. We are looking for enthusiastic, motivated, caring and compassionate individuals who are eager to work with us to maintain the excellent working relationships that have been established with our colleagues across the trust in the delivery of holistic, person centred care and also help us with ongoing innovation and development.

To operate as a Mental Health Liaison Nurse within the Older Adult’s pathway at North Tyneside General Hospital and Royal Quays Intermediate Care Unit.

To work as an integral part of the multidisciplinary ward team providing specialist mental health nursing assessment including risk assessments, clinical care and therapy interventions and be professionally accountable and responsible.

To provide specialist skills, knowledge and experience for those identified as having Mental Health problems who also are also experiencing acute/chronic medical problems.

To provide efficient and effective facilitation of patient journey from admission through to discharge.

To aim to improve outcomes for older people with comorbid physical and mental health problems who are situated in the medical setting.

To enhance and develop the skills/knowledge base of the general hospital staff and improve the quality of care by providing direct support in the planning and delivery of structured education to service users, relatives, carers and other professional staff.

• To provide liaison service as above to the general hospital, intermediate care setting.
• To act as a specialist resource on older persons mental health issues to the general hospital, intermediate care.
• To undertake Specialist Mental Health Nurse role within the broader Mental Health Services for Older People department.
• To operate within a multidisciplinary framework and contribute meaningfully to the development of the service, and the provision of safe and effective care.
• Practice within legal and ethical frameworks encompassed within nursing and mental health legislation to exercise professional accountability as set out in NMC Professional Code of Conduct and work within the scope of practice.
• Reportable to MHSOP Liaison Nurse Lead.
