Summary
Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. For the full description and responsibilities please see the attached job description and personal specification To provide leadership and management (in alignment with the supervision structure) tothe team including undertaking appraisals, supervision, training and development ofbands 2, 3 and 4 staff The post holder will assist the Senior Care Coordinator and Intermediate Care Team Leadto monitor and report on quality, productivity, and patient flow in line with internal KPIs. Where required, to visit patients in the community to ensure quality, support patient flowand manage any issues required, relating to the delivery of care To be responsible for dealing with telephone complaints/ enquiries and othercommunications with a high degree of tact, diplomacy, and confidentiality in line withCPFT procedures and policies.
