Summary
The role will involve an element of 1 -1 personal support for children and young people with an emphasis on enabling them to access additional community based, person centred support that meets their physical, social and emotional needs. This role is primarily to work with children and young people aged 5 -18 years within a holistic, person-centred community-based framework
