About the Role:
As a Care Coordinator, you will oversee a team of support workers in our supported living services, ensuring high-quality, person-centred care. You will manage rotas, liaise with families and professionals, and provide on-call support as needed. Your leadership and organisation skills will help us deliver safe, responsive, and empowering care.

Key Responsibilities:

Coordinate day-to-day operations of supported living services

Supervise and support care staff, providing guidance and mentorship

Ensure care plans are up-to-date and person-centred

Maintain accurate records and monitor quality standards

Take on on-call duties to provide guidance and support out-of-hours

Liaise with families, healthcare professionals, and local authorities

Experience in social care, ideally with adults with learning disabilities

Strong leadership and organisational skills

Ability to make decisions confidently and handle on-call responsibilities

Relevant qualifications in health and social care
