Summary
Job Description Be the main point of contact for NHS referral partners and other organisations into the charitys support offering for people with aphasia of working age in Leeds. Complete in-person assessments for all new referrals in Leeds who meet the eligibility criteria for the project. Oversee three brand new support groups for people with aphasia of working age in Leeds. Ensure outcome measures are completed for each participant on the project and ensure all project paperwork is complete in time for reporting requirements. Provide support and mentoring for the Community Engagement Officer who is new in post. Complete other tasks which are commensurate with the role
