Summary
For further information, please refer to the Job Description / Person Specification Clinical Expert To provide a nursing service within symptom control & palliative care by receiving direct referrals and undertaking assessments based on specialist palliative care needs. To use advanced clinical judgement to plan, implement and evaluate care for patients with complex symptom control needs. To manage an agreed caseload, under the supervision of the Matron and Nurse Consultant, using in depth knowledge of the patient / client group and acting as the key worker where appropriate In collaboration with other clinical nurse specialist to formulate and/or revise relevant policies and to regularly review these following changes and implementation. To work within the procedures and policies of the Trust. To discuss in-depth all care options with sensitivity, knowledge and expertise to enable patients to make an informed choice. To act as patient advocate when appropriate and respecting patient confidentiality and privacy with respect for diverse cultural backgrounds and requirements. To maintain accurate and concise records of clinical service. To work in close liaison with multi-disciplinary teams and to act as the key worker or resource worker as appropriate.
