Summary
Dealing with telephone queries, checking and updating patient information with clinical staff, managers and patients. Support nursing staff with the discharge processes. IT system skills. Team work. Trust Values. Communicating information verbally and in writing, providing routine information for patients and staff, for example discharge, admission, transfers and making appointments May be required to handle barriers to understanding for example anxious patients, relatives, cultural differences, and language or communication difficulties Resolves problems, such as locating notes, patient appointment issues, accuracy of patient information, ward queries and escalating when necessary Organises own workload Organises patient transportation, discharges, appointments and patient admissions
