We work closely with GP surgeries and Primary Care Networks delivering a patient centred medication management model that eases the burden on practices and enhances prescribing quality. We handle everything from prescription signing medicines reconciliation and comprehensive medication reviews to running face to face and virtual clinics while also offering bespoke services like chronic disease clinics quality improvement projects and clinical audits. In this role you will provide safe and effective prescribing within your scope of competence deliver medicines optimisation and structured medication reviews in primary care and act as the key point of contact for your allocated sites. You will use EMIS to manage patient records and clinical coding working independently while maintaining close communication with the wider team to ensure the highest standards of patient care and service delivery. This is a client facing role where your clinical expertise will be matched by your ability to build strong relationships maintain professional independence and spot opportunities to add value to the practices you work with.
