Our Community Support Beds are 24/7 regulated services offering short-term assessment and reablement/rehabilitation to help people regain confidence and maximise independence, with the goal of returning home wherever possible. These services support individuals leaving hospital, requiring urgent community response, or benefiting from reablement in a bedded care setting.
About the Role
As the CQC Registered Manager, you will lead and manage the provision of safe, effective, and high-quality residential assessment and reablement services. You will:
Manage a 24/7 service supporting hospital discharge, admission avoidance, and step-up care.
Lead multi-disciplinary teams to deliver goal-focused reablement and recovery.
Ensure safe admissions and timely support planning aligned with “transfer to assess” principles.
Promote coordinated approaches to planned and unplanned care.
Act as the professional expert for the service, advising stakeholders and managing access.
Drive performance, quality assurance, and financial compliance.
Contribute to strategic planning and service transformation.
Why Join Us?
Be part of a transformational journey in adult social care across Derbyshire.
Help shape a new model of care following public consultation and feedback from health partners.
Play a pivotal role in a new partnership between Derbyshire County Council and Derbyshire Community Health Services NHS.
Lead services that make a real difference in people’s lives every day.
What We’re Looking For
We’re seeking innovative and passionate leaders with:
Substantial experience managing operational reablement adult care services.
Proven success in change management and partnership working.
Strong financial, project management, and quality assurance skills.
Level 5 Diploma in Health and Social Care or Leadership and Management in Adult Care.
Excellent communication, people management, and digital proficiency.
Additional Information
This is an exciting time to join our team as we transition to a new model of care in 2025–2026. You’ll be at the forefront of service innovation, working collaboratively with health and social care professionals to improve outcomes for people in Derbyshire.
