Summary
**Please refer to the attached Job Description and Person Specification, in summary:** Working closely with the multi-agency Health & Justice team, you will receive referrals for individuals in the Health & Justice population and you will. Work as part of the outreach team. Assess service users individual needs. Engage with service users and their families. Offer liaison, joint-working and referrals to other relevant services. Independently undertake face to face and electronic screening to support the identification of multiple vulnerabilities of people who have been in contact with the criminal justice system or immigration centre, such as police custody, courts, and community settings. Working to guidelines. Work within limitations and escalate to Practitioners as appropriate. To support Practitioners, STR Workers and Lived Experience workers and Admin as necessary.
