Summary
Key Responsibilities 1. To be responsible for community or hospital admissions, including liaison with other agencies, service users and carers.2. To evaluate the outcome of nursing interventions to ensure that care plan objectives have been met and that health gains have been achieved.3. To co-ordinate care with the MDT and hospital staff at points of attending appointments, admission and discharge.4. Any interventions must be within the post holders scope of clinical practice ensuring that training is maintained and skills are in line with current guidance.5. To provide advice, practical support and guidance with regards to people who may lack capacity to make decisions about their health.6. To develop risk assessments if needed and appropriate risk management plans.7. To undertake or facilitate assessments of patients before admission.8. To promote awareness of health issues and the development of health action plans with service users and their carers. 9. To support the acute hospital services in the assessment, planning, implementation and evaluation of care plans for service users with highly complex needs.10. To facilitate and promote people with a learning disabilities understanding of their own health needs.11. Promote the active participation of service users, their families and carers in the health care process.12. Contribute in identifying barriers for people with a learning disability accessing health care provisions and promote initiatives to overcome these barriers. 13. To have an understanding of other professional roles, both in the community and within the acute trust, in order to work collaboratively meeting the needs of the person with a learning disability. 14. Provide leadership to hospital staff in preparing people with a learning disability for hospital treatment e.g. consultation, explanations of treatment, using different forms of communication. 15. Advocate on behalf of service users and carers to ensure that clients care needs are met and that all concerns are escalated to an appropriate level. Promote the needs and rights of people with learning disabilities including the right to and consent to treatment relating to their mental capacity.
