Summary

We are looking for a Social Worker to join the Adult Transitions Team.

This role requires a Social Work Qualification with a minimum of 2 years' experience.

About the Team:

The team works to supports young people, typically aged 14 to 25, as they move from children's services into adult social care. The role involves assessing needs under the Care Act, conducting mental capacity assessments, and developing person-centred support plans that promote independence, well-being, and inclusion. You will work closely with families, education, health, housing, and other professionals to ensure a smooth transition, especially for those with learning disabilities or autism.

About You:

To be considered, you must have:

  • A recognised Social Work qualification (Degree/DipSW/CQSW)
  • Minimum 2 years post-qualified experience
  • Previous experience within an Adult Frontline Team
  • A full UK Driver's License is essential
