Summary
- Deliver advanced nursing assessments, care planning, and treatment to patients in their homes, managing complex and high-risk caseloads. - Ensure effective caseload allocation, safe staffing, and prioritisation of urgent visits. - Work collaboratively with GPs, palliative care services, therapists, and social care colleagues to provide integrated, holistic care. - Ensure compliance with safeguarding, infection prevention, NMC standards, and Trust governance frameworks.
