Summary

The Role

We are seeking to recruit a dedicated and experienced Adult Social Care Worker

in the Complex Intervention Team – an integrated Mental Health service with North Somerset Council Adult Care and Avon and Wiltshire Mental Health Partnership Trust (AWP).

You will be working with older with severe and enduring mental illness and dementia providing a strengths based perspective within a multi-disciplinary setting. You will be expected to undertake various duties, including Care Act (2014) assessments and reviews, Work closely with social workers in the team to carry out best interest decision-making, capacity assessments, risk assessments, and provide intervention to maximise independence and wellbeing for the people in our service. Working closely with other professionals and agencies, you will ensure people using our service receive an excellent standard of social care alongside their mental health care. You will work within key legislative frameworks such as the Care Act (2014), The Mental Health Act (1983) and the Mental Capacity Act (2005). In this role, you will undertake a number of diverse specialist or technical activities for adults with care and support needs within frameworks or assessments from other professionals, using skills that would be gained through formal qualifications and/or considerable relevant work experience. Roles at this level are expected to work unsupervised for much of the time, accessing and assessing information independently and can be undertaken by telephone, face to face in the community, office base or in service users home.



Key Responsibilities:

To work within the Complex Intervention team to provide support and intervention for people with severe and enduring mental illness and dementia within the remit of adult social care within the integrated team. To work as part of MDT around the person to promote mental wellbeing, reduce and mitigate risk and promote positive outcomes for service uses.
To work with service users and families/carers to provide advice and support, assess risk and care needs of service users under the Care Act and S117 to develop a framework for care plans/programme of intervention to meet their needs.
With support of senior social workers carry out Mental Capacity Assessments and Best Interest decision making.
To undertake a full range of social care and statutory tasks, provide social care knowledge within the integrated team.
To provide timely review in line with Avon & Wiltshire Partnership and N-Somerset organisational procedures/polices and statutory duties.
This position is exempt under the provisions of the Rehabilitation of Offenders Act (Exceptions) Order 1975 and is subject to an enhanced Disclosure and Barring Service check.



Valid driving licence and own transport, is an essential condition for this role.



This position is currently based at The Coast Resource Centre Worle.

About You

We are looking for an individual who is passionate about working in social care and mental health , and who possesses the following skills and experience:



Essential Criteria:

Vocational qualification or experience to NVQ level 3 or 4 relevant to the service user group or equivalent.
Significant practical experience of working with in social care and Mental Health
Sound assessment skills and ability to analyse eligible needs and think creatively to meet needs under the Care Act and S117
Ability to write clear and concise reports which are consistent and of a high quality.
Ability to work without close supervision and recognize personal and professional boundaries to seek advice when appropriate..
Knowledge and understanding of safeguarding protocols and the threshold for service users and carers.
Ability to travel widely throughout North Somerset and out of area.
Satisfactory enhanced DBS disclosure certificate (relevant applications and checks will be carried out before any job offer is confirmed).
