We are seeking an enthusiastic Band 6Physiotherapistto join theIntegratedStroke Early Supported Discharge & Community Neuro Rehabilitation Service.


The Integrated service provided both stroke rehabilitation & community neuro rehabilitation supporting service users with a variety of complex needs.


This is an exciting opportunity to join an establishedmulti-disciplinary teamcomprisingof Physiotherapists,Occupational Therapists, Speech and Language Therapists, Psychologists, Specialist Nurses,and Neuro Rehab assistants.


This role requires the successful candidate to manage a clinical caseload across theNorthof Cambridgeshire. Their basewill be eitherPeterborough or Fenlanddependent onlocationnearest to home.


The roleinvolvesvisiting patients in their homes, wards,or community gyms.There maybe timeswhen there is arequirement to provide clinical cover elsewhere within Cambridgeshire.The role requiresagileworking oftenusing virtual consultation to manageacaseload. We will provide asmart phone and laptop to support this role.



You will provide comprehensive assessments and deliverevidence-basedspecialistneuroPhysiotherapyprogrammes.You will work as part of a multidisciplinary teaminsupportingpatients with physical, cognitive,and psychosocial needs to achieve their potential.

The ideal candidateshouldbe flexible and adaptable to support service delivery.


The successful post holdermust be able to travel independently across their work locality, have avalid driving licence and access to a car for work purposes.

• To be professionally and legally accountable for all aspects for your own anddelegated work.

• To work within Trust,CSP, andHCPC guidelines and to have a good working knowledge of national and local standards of practice in line with legal and operational requirementsand use own professional judgement to vary from established practices whererequired.

• To undertake holistic assessment of patients with neurological problems (including those with complex presentations and cognitive problems) making use of specialist clinical reasoning skills.

• To use evidence-based practice, to ensure clinical care is effective andappropriate.

• To promote independence and wherever possible to avoid hospital admission and reduce inpatient length of stay to ensure those patients receive theappropriate carein the mostappropriate setting.

• Organise and manage own time, delegating work appropriately.

• Conduct risk assessments and health and safety assessments andincludingimmediatelyreporting any changes/newly identified risks.
