Summary
Your role will be to: To complete holistic mental health assessments with Medication review and prescribing. Undertake the assessment, planning and delivery of direct patient care with minimal supervision, ensuring continuity of practice and working within a multi-disciplinary and multi-agency environment. Communicate decisions about mental health care services, referrals and future support arrangements to people with a mental health care need. Be responsible for accurate recording of all patients support, including assessments, reviews, day to day contact and discharges.
