Summary
The Care Coordinator within a Primary Care Network will focus on providing personalised care based on local needs and health priorities. They work as part of a team to deliver well-coordinated care, especially to vulnerable patients or those with complex health issues. This job requires collaboration with GP practices, the Local Authority and other organisations to ensure patients get the best care at the time. The role will focus on identifying trends, improving service efficiencies and reducing health inequalities by enabling targeted interventions for key patient groups.
