Summary
We are looking for a Social Worker to join the Adult Home First Team.
This role requires a Social Work Qualification with a minimum of 2 years' experience.
About the Team:
The team works to manage and respond to enquiries about adult services by providing clear, informed advice and support to individuals seeking care. It includes assessing needs proportionately, exploring care options, and empowering people to access early intervention and preventative services. The post requires collaboration with individuals, carers, professionals, and service providers, while ensuring accurate case recording and reporting.
About You:
To be considered, you must have:
- A recognised Social Work qualification (Degree/DipSW/CQSW)
- Minimum 2 years post-qualified experience
- Previous experience working within a busy adult social work team is beneficial.
- A full UK Driver's License is essential
