Summary

The Practice Nurse will work as part of the primary care team to provide safe evidence-based and high-quality nursing care to patients. The role involves delivering a wide range of nursing services supporting health promotion and disease prevention and contributing to the smooth running of the practice. Key Responsibilities & Duties: Clinical Care Deliver childhood and adult immunisations in line with national schedules and local policy. Provide chronic disease management and reviews including: Diabetes Asthma COPD Hypertension Cardiovascular disease Undertake cervical cytology in accordance with national guidelines. Provide wound care and dressings including suture removal. Provide contraceptive advice and conduct contraception reviews. Offer a holistic approach to travel health by providing comprehensive advice to patients prior to travel including vaccinations and medications safe sex food hygiene and sun protection Assist in the provision of minor surgery and contraceptive implants. Carry out phlebotomy ECGs blood pressure monitoring and other relevant clinical procedures. Assess, plan implement and evaluate evidence based nursing care for individuals and groups. Ability to monitor and manage maintenance of stock and equipment to include refrigeration and emergency equipment Governance & Safety Adhere to infection prevention and control policies at all times. Maintain the cold chain for vaccines and medicines. Ensure accurate timely and high-standard clinical documentation. Follow practice policies national guidelines and NMC standards of practice. Work in line with safeguarding policies for adults and children. Safely manage medicines vaccines and clinical equipment. Professional Standards Respect patient confidentiality at all times. Work within the scope of professional practice and seek support when needed. Participate in audit quality improvement and clinical governance activities. Engage in continuing professional development (CPD) and mandatory training. Contribute to the development of practice protocols and guidelines. Teamwork & Communication Work collaboratively with GPs ANPs HCAs pharmacists and wider primary care teams. Provide advice and support to patients carers and colleagues. Participate in clinical meetings, multidisciplinary team discussions and case reviews. Contribute to health promotion and patient education initiatives.
