We are seeking energetic, talented, resilient and committed Team Leaders to secure these outcomes. You will have sound knowledge of assessment and care management; have practiced extensively under the relevant legislation; and work in a person centred manner which will be fully embedded in your practice. You will be confident in managing risk and looking for innovative ways to promote independence and link people into the local community.

We have a large range of services that require to work effectively together, we work in full partnership with our health colleagues and a broad range of partners and we have a key priority in relation to shifting the balance of care across both service areas and promoting more effective early intervention and preventative supports.

Applicants must possess a Degree / Diploma in Social Work, Occupational Therapy, Nursing or equivalent qualification and be able to demonstrate commitment to CPD.
