Summary

Work collaboratively with and undertake direction from the Managers/Clinicians in terms of managing and prioritising workload. Be a single point of contact for patients, carers and families and professionals On discussion with the lead clinician, care navigate, signpost and refer patients to the most appropriate service to meet their needs Support the team with the preparation of RESPECT forms Undertake home visits as directed Support the GPs with EOL consultation Perform basic clinical investigations such as ECGs, vital signs and phlebotomy. Carry out some objective and subjective patient assessment and feedback to a registered clinician Give information and support patients and families in regards to care pathways Support family and friends with planning for EOL and bereavement Coordinate MDTs, ensuring they are recorded and actions are coordinated. To have responsibility for appropriate documentation related to patients in their care to be disseminated to all relevant professionals in a timely manner and in accordance with operating standards. To provide, and receive, information with empathy and understanding, and to adapt communication when barriers to understanding arise (seeking senior guidance when necessary). Organise, prepare, and document MDT meetings, completing any required actions When required to oversee and action emails in the 'Care Home Inbox,' ensuring messages are addressed promptly, triaging them appropriately, and assigning tasks as needed; When required, to organise, prepare, and document ward rounds for Care/Nursing and LD home patients, both virtually and in person; Identify urgent patient needs and ensure appropriate action is taken or to connect with the team for advice; Offer advice on self-care techniques and arrange supportive equipment if required; Update and review patient care plans regularly in collaboration with patients, families, and carers; Be a resource to provide information and support for Carers; To coordinate Cancer care reviews; To assist with coordinating and booking Learning Disability and Dementia Annual reviews, working with clinicians, patients and their families, residential/Nursing homes; Complete appropriate referral forms if required; To be an advocate for our vulnerable patients as needed. To develop and maintain effective working relationships with other members of the patients care team To work with the Safeguarding team to develop, update, and monitor safeguarding registers and inbox and ensure alignment with statutory requirements; Ensure requests for Safeguarding information/attendance at related meetings/conferences is managed and acted upon Ensure patient records are updated in relation to changes in safeguarding information Act as a point of contact for safeguarding services, including statutory agencies and other professionals; Maintain safeguarding alerts and coding in patient records and follow protocols for child and adult safeguarding concerns; Manage and respond to safeguarding inbox requests and referrals, ensuring appropriate action is taken in line with organisational policies; Provide updates for the teams in relation to the safeguarding register
