Summary
The post holder will practice as a senior clinician providing visionary leadership within Virtual Ward, UCR, ACP services and SICC that form the Out of Hospital Urgent Community Services Directorate. They will work alongside the GP Extensivists and form part of the senior clinical rota working over 7/7. The Senior ACP will practice independently and autonomously and assume a high level of clinical patients, seeing patients with undifferentiated and undiagnosed problems. The post holder will develop and support a team of Advanced Practitioners with the aim of improving the quality and efficiency of patient focused systems and processes. The post holder will also provide expert clinical and professional advice to staff within the Directorate, Division and across the wider organisation developing strong links with primary care and third sector organisations. . Autonomously act and be recognised as a clinical expert in their specialist field using developed specialist clinical knowledge ensuring patient/client centred care and family involvement, including health promotion and prevention of acute deterioration. Make difficult decisions and encourage critical thinking regarding both ethical and legal issues related to care delivery. To manage own workload (whilst acting as an Independent Prescriber) that often be complex and unpredictable: Receive referrals for Virtual Ward/UCR/ACP and SICC and assess, order, interpret and act upon investigative tests, consider differential diagnosis, evaluate, treat, plan, refer and discharge patients/clients within the Services. Work collaboratively with the VW GP Extensivist within defined clinical pathways and evaluate clinical effectiveness. Direct responsibility for the management of appropriate patients VW/UCR/ACP and SICC -within the Directorate To practice as a senior clinician with an ongoing requirement to act with only minimal guidelines and to set goals and standards for others to follow To sustain intense mental attention and concentration and the ability to respond proactively to the changing needs of the Directorate. To appropriately assess, examine, investigate, diagnose and treat patients within the Directorate services resulting in the safe and appropriate management of patients with undifferentiated and undiagnosed presentations and support a robust framework of clinical competencies for ACPs, and other staff working within the team Maintain own clinical development by keeping abreast of new treatments and technologies and by ensuring care is evidence-based To improve the quality of the patient experience by identifying and meeting the individual clinical needs of patients Exhibit a commitment to new ways of working with Virtual Ward/UCR Service/ACP/SICC To provide specialist advice on patient care to the healthcare team considering all aspects of clinical governance, and act as a specialist resource within the service and the wider Trust To be able to give clinical advice and support the ACPs and other colleagues within the Services To develop and deliver educational training and development to the ACPs and other colleagues on a regular basis. To offer regular clinical supervision and mentorship to ACPs and junior staff. Post holders will work collaboratively with GP Extensivists to support Senior clinical assessment and management of patients admitted to Virtual Ward (Hospital at Home) and Urgent Community Response service. The role is pivotal in providing clinical supervision to our ACPs who also form part of the Directorate and as a collaborative union of services meet our main objective of promoting quality care at home. The shifts for these posts will NOT include 12 hour long days, shifts will work over 7 days and a UK Driving licence is essential. The post will be structured around four core functions that exemplify the role: An expert clinical practice function A professional and operational leadership and consultancy function An education training and development function A research, evaluation, and service development function The Senior ACP will provide highly specialised, expert advice, consultancy, clinical expertise and leadership to patients, carers and Trust staff. They will develop and influence strategy and policy to ensure excellence in clinical care for patients accessing Virtual Ward, Urgent Community Response (UCR), Advanced Clinical Practitioner (ACP) services and Stockport Integrated Care Coordination (SICC) within the vision of the Directorate. If successful this vacancy will require an enhanced DBS check at a cost of £54.90. This cost will be deducted from your first Trust salary In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £16.
