Summary
Key Responsibilities: Conducting thorough assessments to understand the needs and strengths of individuals experiencing early psychosis. Developing and implementing personalised care plans in collaboration with service users, their families, and other professionals. Providing ongoing support and monitoring progress to ensure that care plans are effective and responsive to changing needs. Facilitating access to a range of services, including mental health, social care, and community resources. Advocating for service users' rights and ensuring that their voices are heard in all aspects of their care. Participating in regular team meetings, case reviews, and professional development opportunities.
