Liverpool City Council is urgently seeking an Experienced Adult Social Worker to join our Mental Health team, supporting patient flow and complex discharges from psychiatric units. This is a business-critical role funded through hospital discharge monies, with a clear focus on easing system pressures and improving outcomes for adults over 65.
Your Impact:
Facilitate timely and safe discharges from psychiatric units and step-down arrangements
Support complex care planning for older adults across localities
Collaborate with health professionals, families, and community services
Navigate challenging transitions with empathy, clarity, and resilience
Contribute to a high-performing team during a critical period for adult social care
What You’ll Need: ✅ Professional qualification in Social Work (Diploma, BA/BSc/MA, CQSW) ✅ Registration with Social Work England ✅ Proven experience in adult mental health and hospital discharge work ✅ Strong assessment, planning, and safeguarding skills ✅ Ability to work under pressure and manage complex caseloads
