Summary
Provide advanced clinical assessment, diagnosis, and treatment for patients with acute and chronic conditions Prescribe medications and manage care plans independently Support urgent care and same-day appointments Home visits Support the GPs by carrying out ward rounds to our local care homes Routine Practice Nurse duties Contribute to QOF, audits, and clinical governance Work closely with GPs and the wider team to improve patient outcomes Participate in safeguarding, risk management, and practice development
