Summary
CommunicationsEnsure that communication with patients, relatives and service users is conducted in a courteous, caring, sensitive and appropriate manner, seeking support from the Registered Nurse as appropriate, including contributing to the emotional support of patients who appear to be or are identified as anxious, upset, frightened or worried.Support patients and their families/carers, through communicating routine and non-routine information using a variety of methods, displaying tact and empathy, recognising the needs for alternative methods of communication to ensure effective understanding where there may be barriers to understanding.Contribute to the effective routine communication between members of the multi-disciplinary team, taking personal responsibility to update and share information. Patient Care / Clinical Services Undertake delegated clinical assessments (which will require developed physical skills,speed and accuracy) ensuring that results are given to the Registered Nurse in a timelymanner to facilitate prompt analysis and assessment. Assess and plan care within appropriate boundaries of job role. Assess patient condition through observations/test results and instigate emergencyprocedures as appropriate. Assist patients to undertake normal activities of daily living where necessary andsupports them with fundamental care such as eating and drinking and toileting. Practice safe moving and handling procedures when assisting patients with mobility,using appropriate equipment e.g. glide sheets, which will require physical skills obtainedthrough practice. Proficient and competent using necessary equipment to undertaketasks e.g. using hoists etc. and point of care testing.
