Are you an experienced senior nurse looking to shape and lead high-quality care foradultsin communitynursingsettings? We are seeking aConsultant Nurseto join our BerkshireCommunity Nursing Service, where your clinicalexpertiseand strategic leadership will make a real difference.


AsConsultant NurseforCommunity Nursing, you willbe responsible forsupporting, leading, and developing advanced nursing practice across our physical healthCommunity nursingservices.Working at a strategic level, you will collaborate closely withour Primary Care colleaguesand multidisciplinary teams including nursing, therapy, andAcutemedical colleagueswhilstdelivering safe, effective, and person-centred care.


You will lead on clinical innovation and excellence in patient care, provide strategic and operational leadership, and work flexiblyCommunity Nursing teamswithin Berkshire. You will spend at least50% of yourtime in direct autonomous clinical practice, with clinical responsibility for a defined cohort of patients and their carers’ and will provide leadership for clinicians within Community Nursingservice.You will alsochampionbest practice through audit, research, the application of national policy, and act as a key clinical leader and role model for staff across the service.







Advanced and Expert Level Practice

• Lead advanced, autonomous clinical care for adults living with frailty, including assessment, prescribing, and care planning.

• Maintain expert clinical skills and provide direct patient care (minimum50% of time).

• Support safe, evidence-based practice across inpatient wards and promote patient-centred care in partnership with families and carers.

• Actively contribute to multidisciplinary team (MDT) meetings and apply advanced clinical reasoning in complex cases.


Consultancy and Leadership

• Lead the development and implementation of frailty care models across communitynursingsettings.

• Act as a senior clinical leader, offering expert advice, driving quality improvement, and promoting patient safety.

• Represent the service in internal and external reviews, legal proceedings, and trust-wide initiatives.

• Provide compassionate leadership, coaching, and mentorship to support team development and workforce sustainability.


Education and Training

• Create a learning culture that supports professional development for nurses and students.

• Collaborate with education providers to develop and deliver training on frailty, ageing well, and clinical excellence.

• Promote evidence-based practice and enhance the clinical competence of inpatient staff.

1.Registered Nursewith the NMC


2.Teaching qualification with recent experience of delivering training


3.Advanced history taking and physical examination with clinical reasoning qualification


4.Non-medical prescribing


5. Leadership & management qualification


6.Previousexperience withinolder adultcare.


7. Advanced and effective leadership/communication skills, written & verbal.


8. UK driving licence
