Summary
Deliver safe, effective, and evidence-based clinical care to patients across the PCN. Undertake consultations (face-to-face, telephone, or online), including assessment, diagnosis, treatment, and follow-up. Manage both acute and long-term conditions, providing continuity of care and personalised management plans. Prescribe appropriately in accordance with national and local guidelines. Undertake home visits where clinically required. Provide timely referrals to secondary care and community services as appropriate. Contribute to the PCNs contractual service delivery (e.g., Enhanced Access, IIF, QOF, and Directed Enhanced Services). Work collaboratively with the multidisciplinary PCN team (e.g., pharmacists, paramedics, social prescribers, care coordinators, health and wellbeing coaches) to ensure holistic care. Support proactive population health management, addressing health inequalities and improving patient outcomes. Maintain accurate, comprehensive, and timely clinical records in line with GMC and NHS standards. Engage in audit, clinical governance, and quality improvement activities to support safe and effective practice. Participate in PCN and practice meetings, contributing to service development and innovation. Provide leadership, supervision, or support to other ARRS staff and trainees where appropriate. Keep up to date with mandatory training, appraisal, and CPD requirements to maintain GMC registration and revalidation.
