Your Role Will Include:
Delivering personal care and one-to-one support in home and community settings
Supporting individuals to achieve personal goals and engage in meaningful activities
Following personalised care and support plans
Promoting independence, wellbeing, and social inclusion
Maintaining accurate records and supporting health assessments
Upholding best practices in safeguarding, health & safety, and mental capacity
